At Granite Bay Dermatology and Laser Center, we are devoted to the health of your skin.

Welcome!

We want your visit to our office to be a pleasant experience. Above you'll find links to information regarding your visit. Please fill out all forms as completely and accurately as possible. This will allow us to have the necessary information to provide you with the highest level of care.

1. 1. Before you visit, please call our office at 916-773-3376. Our receptionist will provide you with a special code to access our New Patient Registration.
2. 2. Log in to our New Patient Registration with the code you were provided. Please fill out all forms as completely and accurately as possible.
3. 3. Please print all pages under “important patient information” above and sign all pages that require a signature.
4. 4. The HIPAA Privacy Notice is for your reference and does NOT need to be printed.

We ask that you please complete this information prior to your visit and bring everything with you to your appointment. If you are not able to do this, please plan to arrive at least 15 minutes prior to your appointment time. You can also download directions to our office. Please bring these directions with you when coming to your appointment as this will be necessary to find the exact locations of our building.

If you find that your need to cancel or reschedule this appointment, or any subsequent appointment, we ask that your contact our office at least 24 hours in advance. Patients scheduled for a procedure should provide us with at least 48 hours notice prior to canceling or rescheduling an appointment to avoid forfeit of deposit. This allows those who are waiting for an appointment the opportunity to be scheduled in your place.

We love children. They are innocent, naturally curious, and active. These charming qualities often require undivided attention from adults. Limiting the number of people at your visit or your child's visit allows us to focus our attention on the patient. Please make every effort to arrange supervision for your children who do not need our medical care. Also, please note that all patients who are under the age of 18 must be accompanied by a parent or legal guardian to receive treatment. If a follow-up visit is necessary, a form may be signed by the minor's parent or legal guardian that gives us permission to treat an unaccompanied minor.
